- This form you are to have all students in your department sign along with parents. You are to maintain this form in your department files. Do Not send these forms to the state office.
MO FFA Chapter Assurance Form
- This form is to be completed by the advisor with signatures from the advisor (s) and administration and then sent to our office.
- After obtaining signatures please name your assurance form file – Chapter Name Assurance Form 24-25 so the example would be Polo FFA Assurance Form 24-25
- Emailed to the state department using the moffa@dese.mo.gov email with the subject line: 24-25chapternameassuranceform – example: 24-25 East Prairie Chapter Assurance Form
- Please consider making this a routine part of your beginning of the year paperwork that needs to be completed every fall.
